You need to do a MRI of the pelvis. The MRI will need to be done with contrast media. Please ask your doctor to request for the following sequences:

*T2W1: sagittal ( fat-suppressed and non-fat-suppressed), transverse, coronal
*T1W1: sagittal, transverse
*T1W1: contrast-enhanced (if no contraindication): sagittal, transverse
DWI (if possible):transverse

Please call us and get an appointment. We will like to see you early in the morning so that we have time to arrange for the MRI.

You need to do bowel preparation as follows:

3 days before HIFU
Do not eat: fruits, vegetables, red meat, brown rice, chilli, corn, oatmeal, nuts, wheat, bread
Can take: Rice, noodles, chicken, fish, pork, ice cream, yogurt (without fruit or nut)

2 days before HIFU
Do not eat: fruits, vegetables, red meat, brown rice, chilli, corn, oatmeal, nuts, wheat, bread, any food with particles of dried food or seasoning, avoid milk and cream and alcohol
Can take: clear soup, porridge, water, coffee, tea

1 day before HIFU
Do not eat: fruits, vegetables, red meat, brown rice, chilli, corn, oatmeal, nuts, wheat, bread, any food with particles of dried food or seasoning, avoid milk and cream and alcohol
Can take: clear soup, watery porridge, water, coffee, tea

Dr Selva will then take a history and perform a physical and pelvic examination. He will also perform a pelvic and abdominal ultrasound. If you have not done the MRI it will be done at Mahkota Medical Center.

After reviewing the MRI, he will then arrange for a simulation of HIFU. Here you will be placed on the HIFU machine to see whether you are suitable to undergo HIFU treatment. After that if you are suitable for HIFU, we will arrange for a date for the HIFU treatment. We will then apply for your insurance coverage and give you instructions on your diet before the day of the HIFU surgery.

  1. You will usually be admitted 1 day before the surgery during the HIFU surgery.
  2. You and your partner will require a COVID test.
  3. You will have a blood test.
  4. You will be given a medication (FORTRAN) to drink at 6 pm and 8 pm the night before.

The surgery is usually done in the afternoon.

  1. You will be given a enema on the day of the surgery
  2. A few hours before the surgery, a intravenous drip (IV drip) will be set up
  3. A urinary catheter will be placed in the bladder
  4. You will be brought to the HIFU centre about 30 minutes before the procedure
  5. Your abdomen will be cleaned and then you will be placed on the HIFU table before the surgery is performed
  1. You will be rested in the prone position for 6 hours
  2. After that you can get up and walk
  3. After 2 hours you are allowed to drink clear fluids.
  4. Keep your vulva clean
  5. Most of the time you will be able to go home the next day
  6. Some patients may have to stay for an extra day.
  1. You should take it easy for a few days

  3. No sex after treatment: you can resume after one menstrual cycle
  1. Pain at the lower abdomen, sacrum area and legs
  2. Temporary weakness of the legs
  3. Redness of skin in the treatment area
  4. Dizziness due to medication, low bleed glucose lever etc
  5. The first and the second menstruation after treatment may increase or lengthen.
  6. It usually takes 3 months for symptoms to improve significantly
  7. Patients with submucous fibroids may have secretions lasting about one week after treatment. Keep the vulva clean and take oral antibiotics 3-5 days to prevent infection
